Annual or Monthly Subscription
FormPro is a professionally-designed product offered as a subscription renewable on an annual or monthly basis. There are many advantages to a FormPro subscription:
  • All software updates are included. You will never be asked to pay an additional fee for software updates while your subscription is active.
  • All data updates are included. FormPro automatically downloads new reference and medical treatment data every time you start the program. You will never be asked to pay for additional data or information while your subscription is active.
  • Technical support is included. You will never be charged a fee for technical support while your subscription is active. Call us whenever you need assistance; no problem is too small!
  • Easy budgeting. Since there are no additional or hidden costs associated with FormPro, your organization can plan for a subscription without concern for any unforeseen expenses.

Below is a list of injury categories included in FormPro. The list will grow over time as new data are added to the system.
  • Amputation: available now and includes upper extremity and lower extremity
  • Brachial plexus injury: available now and includes intermediate plexus palsy (C7 +- C8); lower plexus palsy (C8 and T1); total plexus palsy; and upper plexus palsy (C5, C6, +- C7)
  • Burns: available now and includes electrical;head and neck; inhalation; lower limb; trunk, pelvis, and/or perineum; upper limb
  • Cerebral palsy: available summer 2010
  • Complex regional pain syndrome: available now and includes upper extremity and lower extremity
  • Hearing impairment: available fall 2010
  • Joint injury: available summer 2010
  • Spinal cord injury: available now and includes anterior cord; Brown-Sequard; cauda equina syndrome; central cord; complete paraplegia; complete tetraplegia (quadriplegia); conus medullaris; incomplete paraplegia; incomplete tetraplegia (quadriplegia)
  • Spinal column injury: available fall 2010
  • Traumatic brain injury: available now and includes locked-in state; mild; minimally conscious state; moderate; persistent vegetative state; severe
  • Visual impairment: available fall 2010
